CMHA Ottawa – Library Book Request

CMHA Ottawa has a large selections of books for staff to borrow and read.  In order to begin the process of checking out a book please fill in the form below and click submit.

You request will be sent to the CMHA Learning & Development team and we will contact you once we have reviewed your request to confirm your pick up date.  All borrowed materials will be placed in your mail slot for pick up and we need 2 business days from the time of your request to ensure the book is available and ready for pick up.

At the end of your 4-week borrowing term, we ask that you alert both Julie Levesque and Helen Gottfried-Unruh once you have placed the returned book in Helen’s mail slot.  At this time we will consider the book returned.
